£27,165 per annum - 39 hours per week

Treloar’s School and College is looking to recruit a Progress and Transition Coordinator to provide a robust, age appropriate, person-centred transition and future planning for students and their parents. The successful candidate will lead the student’s multi-disciplinary teams, and will work with students throughout their journey with us and for a period of time beyond.

The post holder will be responsible for a caseload and leading the multi-disciplinary team meetings, monitoring progress, engaging with students of the school and college and most crucially students who are at key transition points in primary, secondary and post 16 provision. Duties will include liaising with external agencies, preparing, attending and contributing at review meetings, representing Treloar at external meetings, developing transition plans, visiting students in their own homes and supporting students, families and the multi-disciplinary teams with Education Health and Care Plan processes. You will be responsible for coordinating external work placements.

The post holder must have good general education to Level 3 or above and must hold a Level 2 qualification in Numeracy and Literacy. You must also have a Level 4 qualification in Information, Advice and Guidance (or similar) and a Level 3 qualification in Health and Safety or be willing to commit to achieving these.

Hours: 39 hours per week (full time) There is a requirement to work 15 days outside of term time to visit students / families in their own homes or placements.
