Transition
Our Progress and Transition team work collaboratively with families, students and external stakeholders to prepare young people for life beyond Treloar's.
All students are allocated a Student Progress Coordinator (SPC) who will work with the multi-disciplinary team (MDT). SPCs lead weekly multi-disciplinary teams meetings and monitor progress, ensuring appropriate targets are set against EHCP outcomes in line with students’ aspirations.
The SPC will also provide advice and guidance on living, activities, further learning and future placement options, supporting the young person, their family and external stakeholders with this process. The SPC will support students and families in Annual Reviews and, where relevant, will link with parents and agencies in a student’s home area.
Our Former Students Officer tracks young people post-Treloar's up to 10 years providing comprehensive destination data to support the ongoing development of our curriculum. A large percentage of our Alumni each year have achieved their aim of placement. Moving onto a range of destinations for example, supported living managing their own PA’s, voluntary/paid employment, higher education and residential provision having increased choice and control over their environment.
Where appropriate, we provide opportunities for work experience placements either internally at Treloar’s or externally. The aim is to give students personalised experiences, knowledge, skills and planning tools which will enable them to make informed decisions and choices about future, paid or unpaid, employment. This is aimed at preparing them for when they leave College.
